RESUMEN

The elucidation of the C-arylation of active methylene compounds under visible light conditions without a photocatalyst presents an intellectual challenge. The photo-induced C-arylation of active methylene compounds via electron donor-acceptor (EDA) complexes shows a strategic path for the synthesis of pharmacologically relevant compounds. This expansive and efficacious methodology facilitates C-arylation under environmentally conscientious conditions, exhibiting exemplary compatibility with diverse functional groups and yielding numerous compounds. This environmentally sustainable transformation underscores the merits of the procedural simplicity and benign reaction conditions. Notably, all resultant products were judiciously derived from active methylene compounds and diazonium salts through the intermediacy of the EDA complex.

BACKGROUND: Pharmacovigilance (PV) deals with the detection, collection, assessment, understanding, and prevention of adverse effects associated with drugs. The objective of PV is to ensure the safety of the medicines and patients by monitoring and reporting all adverse drug reactions (ADRs) associated with prescribed medicine usage. Findings have indicated that about 0.2- 24% of hospitalization cases are due to ADRs, of which 3.7% of patients have lethal ADRs. The reasons include the number of prescribed drugs, an increased number of new medicines in the market, an inadequate PV system for ADR monitoring, and a need for more awareness and knowledge about ADR reporting. Severe ADRs lead to enhanced hospital stays, increased treatment costs, risk of death, and many medical and economic consequences. Therefore, ADR reporting at its first instance is essential to avoid further harmful effects of the prescribed drugs. In India, the rate of ADR reporting is less than 1%, whereas worldwide, it is 5% due to a need for more awareness about PV and ADR monitoring among healthcare providers and patients. The main objective of this review is to highlight the current scenario and possible futuristic ways of ADR reporting methods in rural areas of India. We have searched the literature using PubMed, Google scholar, Indian citation index to retrieve the resources related to ADR monitoring and reporting in India's urban and rural areas. Spontaneous reporting is the most commonly used PV method to report ADRs in India's urban and rural areas. Evidence revealed that no effective ADR reporting mechanisms developed in rural areas causing underreporting of ADR, thus increasing the threat to the rural population. Hence, PV and ADR reporting awareness among healthcare professionals and patients, telecommunication, telemedicine, use of social media and electronic medical records, and artificial intelligence are the potential approaches for prevention, monitoring, and reporting of ADRs in rural areas.

Henoch-Schönlein purpura (HSP)/immunoglobulin A vasculitis is an acute immunoglobulin-mediated vascular disorder compromising a triad of non-blanchable purpuric rashes, colicky abdominal pain and haematuria. Incidence ranging for large populations has been most common vasculitis among children with a mean age of 6years. Disease presentation ranges from small petechial rash to the most severe form of renal involvement so early diagnosis is important. Case presentation: Here the authors present a 4-year-old girl with antecedent allergic history associated HSP. She presented with non-blanchable purpuric rashes, abdominal pain with bilateral ankle swelling. She had been treated for atopic dermatitis and acute urticaria without angioedema and acute tonsillitis. The potential association of HSP and these condition should be kept in mind in order not to miss the impediment complications of HSP. Clinical discussion: HSP usually presents with rash, abdominal pain and vomiting, joint pain especially involving knees and ankles, subcutaneous oedema but in severe cases there could be renal involvement leading to nephritis, neurological manifestation involving neuropathies, altered mental status and many others. Conclusion: This case highlights the rarity of disease, presentation, association with allergic diseases and availability of treatment modality.

RESUMEN

An efficient C-N cross-coupling approach for the synthesis of hydrazones was developed through C(sp2)-H and C(sp3)-H functionalization of indole and methylarene under visible light irradiation using photocatalyst eosin Y, ethanol:water as a green solvent and atmospheric air as an oxidant. With the aid of eosin Y, the C-H bonds of indole and methylarenes were activated followed by coupling with arylhydrazines. The procedure was applied to a wide variety of substrates with good functional group compatibility, offering a creative way to make hydrazones from inexpensive and easily accessible raw materials. The absence of metals, low cost, environmental friendliness, green solvent, non-toxicity, ease of handling, and utilization of renewable energy sources like visible light are some of this method's primary advantages.

A 9-year-old male patient presented with recurrent episodes of corneal ulcers in both eyes since the age of 2 years. The patient had profound bilateral hearing loss and multiple left-sided preauricular skin tags. He was diagnosed with neurotropic keratitis with bilateral corneal anesthesia in both eyes and on further work-up was diagnosed to have Goldenhar Syndrome with features of left-sided facial skeletal hypoplasia and bilateral auditory nerve atresia. The patient was managed by multiple specialities with standard of care medications, therapies, and procedures and is presently thriving. Goldenhar syndrome is a rare congenital disorder which involves ocular, auricular, and cranial nerves along with facial and vertebral anomalies. The insufficient knowledge of its pathogenesis and variable clinical presentations present a challenge in timely diagnosis and management of these cases. The involvement is generally unilateral and unilateral skeletal asymmetry with bilateral cranial nerve involvement is a rare event. The wide variation in clinical presentation and under diagnosis of these cases in India mandates a better understanding of this entity among medical professionals to facilitate early diagnosis and favorable clinical outcomes.

The present study focuses on the possibility of applying fly ash to agricultural fields for enhancing the production of agricultural crops. In this study, Pisum sativum L. was grown from germination stage to maturation stage in phytoremediated and non-phytoremediated or raw fly ash-amended soil. All the morphological (height, biomass, number of leaf, and leaf size) and physiological parameters like, protein content, chlorophyll content, nitrate reductase activity, and peroxidase activity were monitored to understand the effects of fly ash or its usefulness for using it as a fertilizer for facilitating micronutrients. Major finding of this study is that 40% (w/w) of non-phytoremediated fly ash amendment could be used for field application. Percentage increase of toxic metals in below ground organs was 6% for Cd, 6% for Cr, 5% for Cu, 15% for Mn, and 7% for Pb when compared with the control. In the non-phytoremediated fly ash-amended set, heavy metals and metalloids were present in the grains only at higher amendments T3 (60%) and T4 (80%). However, except Cd, all the metals were below the permissible limits suggested by the WHO. Phytoremediated fly ash could be used as a fertilizer up to 100% for the cultivation of pea plant as metals concentrations were found either below detection limit or below the WHO permissible limit.

BACKGROUND: Spectral domain optical coherence tomography (SD-OCT) detects early structural damage to macula in patients on Hydroxychloroquine (HCQ) therapy. The current screening protocols emphasize concurrent use of both SD-OCT and Visual field analysis (VFA) which detects functional damage to detect Hydroxychloroquine maculopathy. However, VFA is a time-consuming and subjective test which is often neglected. This study gives the prevalence of Hydroxychloroquine maculopathy using SD-OCT alone which often fails to detect macular damage in peri-foveal and extra-foveal area of the retina. METHODS: Three hundred thirty four eyes of 167 patients taking systemic Hydroxychloroquine were studied with SD-OCT macular cube 512 x 128 to diagnose structural macular damage to detect prevalence of Hydroxychloroquine maculopathy. RESULTS: Out of 167 patients, only four patients showed features suggestive of Hydroxychloroquine maculopathy. One patient had ELM loss, two had para-foveal and one had peri-foveal IS-OS disruption. The SD-OCT gave prevalence as 2.4%. CONCLUSION: SD-OCT alone can underdiagnose burden of Hydroxychloroquine maculopathy (prevalence = 2.4%) when compared to studies which use both SD-OCT and Visual field analysis (prevalence = 7.5%).

The present work had two purposes firstly to evaluate the potential of Lantana Camara for phytoextraction of heavy metals from fly ash amended soil and to assess the suitability of a proper biodegradable chelating agent for chelate assisted phytoextraction. Plants were grown in manure mixed soil amended with various concentration of fly ash. Two biodegradable chelating agents were added (EDDS and MGDA) in the same dose separately before maturation stage. Sampling was done at different growing stages. The plant took up metal in different plant parts in the following order: for Cu, and Zn leaf >root >stem, for Cr and Mn leaf>stem >root, for Ni root >leaf>stem and for Pb root≈leaf>stem respectively. For Cu, Zn, Cr and Mn Lantana camara acted as phytoextractor. Translocation factor and bioaccumulation coefficient was>1 signifying enrichment and translocation of metals in the plant. Morphological studies showed no toxicity symptom in the plant. Among biochemical parameters protein and nitrate reductase activity decreased, whereas, chlorophyll and peroxidise activity increased with the growth stages. Finally, it was evident from the results that Lantana Camara can be used as efficient phytoextractor of metals, with proper harvesting cycle and both chelate were proved as effective chelators for phytoextraction of metals.

Three naturally growing plants Ipomoea carnea, Lantana camara, and Solanum surattense were found in fly ash dumpsite of Patratu thermal power station, Jharkhand, India. They were assessed for their metal uptake potential. The fly ash was slightly alkaline with very less nitrogen and organic carbon but enriched with phosphorus and heavy metals. Lantana camara and Ipomoea carnea showed good translocation from root to shoot for most of the metals except Mn and Pb. The order of metal accumulation in stem of both the plants were Fe(205mg/kg)>Mn(65mg/kg)>Cu(22.35mg/kg)>Pb(6.6mg/kg)>Cr(3.05mg/kg)>Ni(1 mg/kg)>Cd(0.5 mg/kg) and Fe(741 mg/kg)>Mn(154.05 mg/kg)>Cu(20.75 mg/kg)>Pb(6.75 mg/kg)>Ni(4.0 mg/kg)>Cr(3.3mg/kg)>Cd(0.05mg/kg), respectively. But Solanum surattense accumulated most of the metals in roots. The order was in the following order, Mn (382.2mg/kg) >Fe (264.1mg/kg) > Cu (25.35mg/kg) >Pb (5.95 mg/kg) > Ni (1.9 mg/kg) > Cr (1.8mg/kg) > Cd (0.55 mg/kg). The order of Bioconcentration factor (BCF) in root and shoot followed almost the same order as, Mn>Fe>Ni>Pb>Cu>Cr≈ Cd in all the three species. ANOVA showed significant variation in metal accumulation by root and stem between the species. Finally, it can be concluded that Solanum surattense can be used as phytostabilizer and other two species as phytoextractor of metal for fly ash dumpsite reclamation.
